Methods: 30 patients scheduled for selective intracranial aneurysm clipping were divided into 3 groups randomly. They were induced with propofol, fentanyl, and vecuronium and maintained with desflurane inhalation (1.0MAC, 1.3MAC, 1.5MAC respectively). Mean arterial pressure (MAP) and heart rate (HR) were monitored continuously during the surgery. Plasma concentrations of AT-I, AT-II and ETwere measured with radioimmunoassay before induction, during dura opening, during aneurysm clipping and 30 minutes after clipping, respectively. Results: Plasma concentrations of ET increased significantly during surgery compared with baseline in all groups (p<0.05). AT-I increased significantly compared with baseline during dura opening in 1.0MAC group and 1.5MAC group (p<0.05). In 1.5MAC group,AT-I increased significantly during surgery compared with baseling and the data in 1.0MAC group and 1.3MAC group (p<0.05). AT-II had no significant changes within 1.0MAC group and 1.3MAC group (p>0.05). AT-II increased significantly during surgery compared with baseline in 1.5MAC group (p<0.05). Conclusions: 1.0MAC and 1.5MAC desflurane are suitable in intracranial aneurysm clipping with stable hemodynamics, decreased ET and similar plasma concentrations of AT-II to baseline. Key Words: Desflurane; Endothelin; Angiotensins; Intracranial aneurysm 颅内动脉瘤破裂蛛网膜下腔出血导致脑血管痉挛的发病率和死亡率很高。可致血管收缩的内皮素(ET)和血管紧张素I、II(AT-I、AT-II)在脑血管痉挛中起一定作用。目前尚无地氟醚麻醉应用于颅内动脉瘤夹闭术中对ET,AT-I,AT-II影响的报道。本研究拟在颅内动脉瘤夹闭术中应用不同浓度地氟醚麻醉,观察血浆ET、AT-I、AT-II的动态变化。 |
