Department of Anesthesia, Guangzhou Red Cross Hospital of Jinan University School of Medicine, Guangzhou 510220, China Abstract Objective:To investigate effective methods of intractable advanced cancer pain that could not be relieved effectively by the three ladder approaches. Methods:62 cases of intractable advanced lung cancer, liver and pancreas cancer, uterine cancer were pain-relieved by the methods of extra-three ladder approaches?patient controlled intravenous analgesia(PCIA) with description of morphine 0.8mg/ml,droperidol 0.1mg/ml and ketamine 2mg/ml,oral amitriptaline(50-100 mg/d) and psychology treatment. Visual analogue scale(VAS ) and Emotional scale(ES) were used to evaluate pain extent and emotional state of patients before and after treatment. Result:VAS was 7.6, ES was 8.3,sleep and life quality of patients were very lower before PCIA treatment,patients were in serious depression as well and VAS was decreased to 2.3 and ES was 3.6,sleep and life quality of patients were improved markedly after PCIA treatment,excellent and good percent of pain-relief was more than 90% Conclusion:After PCIA treatment with morphine,droperidol and ketamine, intractable advanced cancer pain that could not be relieved effectively by the three ladder approaches was well controlled and life quality of patients were remarkably improved.
