中国人民解放军第九四医院重症医学科 南昌 330002 目的:将量子点探针与流式技术结合,建立定量检测血清降钙素原
(P C T)的新方法并对其检测性能及临床应用进行初步评价。方法:将
羊抗P C T抗体与微球偶联制备诊断微球,与血清或标准品中的P C T结合,
加入鼠抗P C T抗体,最后加入生物素化的羊抗鼠I g G抗体和链霉亲和素化
量子点,流式细胞仪检测平均荧光强度(mean fluorescent intensity,
M F I)。采用前瞻性研究方法,选择2012年1月至2012年8月,入住本院重
症监护病房(I C U)77例老年创伤患者,通过本实验创建的定量检测P C T方
法对77例老年创伤患者入院第1d、3d、5d、7d、10d进行血清P C T检测,
并记录当日急性生理学与慢性健康状况评分系统(APACHE Ⅱ)、C-反应
蛋白(C R P)等,比较P C T水平、A P A C H EⅡ评分及C R P水平,分析各指标之
间的相关性。探讨对老年创伤患者早期感染诊断价值。结果:自创法检
测PCT的灵敏度为5.42pg/ml、线性范围为45.42pg/ml-10ng/ml,针对老
年群体阳性下限值为0.56n g/m l。采用自创法检测P C T水平预测老年创伤
患者发生感染的灵敏度为100%,特异度为24.5%,准确度为84.4%,P C T
水平监测老年创伤病情和感染预测方面优于CRP且与APACHE Ⅱ评分相关
性好。结论:本实验创建的定量检测血清P C T的方法可作为临床检测血
清P C T的方法,早期诊断及监测老年创伤患者合并感染的灵敏度、特异
度较高,适用于临床诊断。

Establishment of a Quantum- Dot Cytometric Assay for Detecting
PCT and the Value of Clinical Detection

Abstract
Objectives. We purified quantum-dots fluorescence probe used in cytometric assay, We aim to establish a quantum
dots cytometric assay for detecting PCT and study its precision, linearity, sensitivity.
Methods. Microsphere surface were labeled by sleep against PCT antibody, combined with PCT, adding rabbit against
PCT antibody,last added biotinylation sleep against rabbit IgG antibody and QDs-SA conjugate. The mean fluorescent
intensity(MFI) were detected by flow cytometer. Elderly-patients with heavey-hurt who enrolled in ICU were retrospec-
tively investigated and analyzed,using the new assay for detecting PCT.
Results. The sensitivity(45.42pg/ml), inearity(45.42pg/ml-10ng/ml) of quantum-dots cytometric assay were better.
The Positive limit is 0.56ng/ml. A method to detect PCT prediction elderly trauma in patients with the sensitivity of infec-
tion was 100%, respectiely is 24.5% ,the accuracy is 84.4%, PCT better than CRP ,has a good correlation with APACHE Ⅱ.
Conclusions. The method can be used as clinical detection serum PCT method, can diagnosis and monitoring old
trauma patients merge infection sensitivity, specific degrees higher, the suitable for clinical diagnosis.
